Below are forms to help you prepare for your Wood Badge experience. Use the scroll bar to the right to view the entire list. If you have not already done so, the Personal Health and Medical Record Form and Personal Resource Questionnaire should be sent to your Course Director as soon as possible.
The Pre-course Assignment should be completed and brought to the first pre-course meeting (see Course Calendar).
