Community Partners, the Living at Home/Block Nurse Program of Two Harbors, works to enable older persons to remain independent in their own homes.  We accomplish this by organizing neighborhood volunteers and providing support services, information and coordination between health and social services.  Community Partners was initiated and developed by area residents.
