(This is the bishop of the ward, congregation, that covers the area you live in. If you do not Know his name just write--bishop, And then the address.  It can usually be found in the yellow  Pages of your phone book under "churches".  If you are not sure call one of the numbers, you don't have to tell them why you want the information.)

Dear Bishop (*INSERT HIS LAST NAME IF YOU HAVE IT):

    (I was baptized a member of the LDS church on ___ in the __________Ward, _________________Stake.)  I want you to remove my name from the rolls of The Church of Jesus Christ of Latter-day Saints.  I no longer consider my self a member of this church. (I expect that you will NOT excommunicate me as I have done nothing to warrant this.  From now on I do not want anyone to visit me or call me on the phone.  I do not want any contact with you and/or your members. If there is a "Bishop's Court" I do not want an invitation nor will I attend it.).

  I expect to have a letter from you in 4 to 5 weeks showing that my name has been removed from the Church rolls.  Thank you for your prompt handling of this.

*NOTE:  Please revise this suggested resignation letter format to fit your needs.  If there is more than one of you then be sure to change the pronouns (I & me to we & us). After you send your letter in you may get someone that will try to block your request.  This can take many forms.  The most prevalent is to just ignore your letter, and another is to say they have to wait for your records to come in.  This may or may not be the case and you will have to determine if someone is stalling you.  An attempt may also be made to try to talk to you.  Do this if you feel very comfortable in doing so, it is YOUR CHOICE, NOT the Bishop's or someone he may send.  I have heard of cases where only under the threat of a lawsuit was action taken on the letter sent.  In some cases more than one letter was needed.  Keep in mind it is your choice to resign, not the Mormon Church's. If you are delayed and stalled it is also your choice to push it as hard as you want.  But in all cases be polite, but firm.
     Also include the Ward, Stake and date of your baptism.  This probably will be needed if you no longer live in the area where you became a Mormon or do not know the Bishop.

Contact person: Gregory W. Dodge, Supervisor, ( maybe he has move on and the new contact is a Mr. Harorough as of 2/9/99),  Phone 801-240-3500 Ext. 1462.   Explain that you've already contacted your bishop and stake president, and that they have not been responsive. (Be sure to mention the name of your ward/stake.)  At the bottom of the letter put a cc: with the names of your bishop and stake president, and send copies of the letter to each of them. This will put them on notice of the step you are taking to others. Others I know of have found that the SLC Membership Records Dept. is very cooperative in situations like this.
